Our Services
As an advocate of family caregivers, we realize that in order to reach family caregivers we cannot do this alone. Caregivers’ Reach provides caregivers with accessible support services throughout the caregivers’ level of importance. The trends that family caregivers face daily may be common as a whole; however, each family’s dynamics and situation is unique. At times, caregivers’ level of concern is based on the degree of a crisis or resolution of a problem.
Caregiver workshops
Caregiver workshops provide family caregivers with helpful resource information and self-care education and provide participants with accessibility to various healthcare organizations.
Summer Youth Intergenerational Technology Program
In collaboration with the Summer Youth Employment Program and AARP Peachtree Chapter 4997 in Co-op City. Seniors receive one-on-one technical instructions on their phones, laptops, Kindles, and computers. The interpersonal interaction between the youth and seniors enables both to learn new things about themselves and life experiences.
In panel Discussions
In panel Discussions, which are organized in collaboration with other community organizations, caregivers receive information from healthcare professionals and participate in a question-and-answer session.
Corporate Training
Provides resources and tools to successfully implement a Family Caregivers’ Support Model for organizations and/or Human Resource Departments interested in establishing support caregivers services for their employees. As part of the COME Network, we develop or enhance an exclusive caregiver support model for organizations.
Telephone Consultations
Telephone Consultations enable caregivers to examine their circumstances more closely, establish a starting point, build confidence, and acquire basic knowledge about the resources available. Self-care, stress reduction activities, and steps to avoid caregiver burnout are also addressed. This service is available on an ongoing basis.
Caregiver Consultation
Caregiver Consultation is a one-time in-person family meeting with all involved stakeholders, including long-distance caregivers. Its purpose is to assess the current plan of care and establish a long-term plan of action that will ensure the patient’s needs are met in a safe environment. The consultation can also include referral to other community resources.
