Services for Family Caregivers
Helping You Navigate the Caregiving Journey
Caregiving needs change over time. Caregivers Outreach provides practical information, community connections, education, and supportive programming designed to help caregivers feel more informed, connected, and prepared.
Resource Navigation & Referrals
Finding the right help can be challenging. We connect caregivers and families with community programs and services based on their individual needs, including aging services, home care, respite, benefits, transportation, food assistance, housing, disability services, mental health resources, legal assistance, and other supports.
Caregiver Education
Being informed can make caregiving feel more manageable. Through educational workshops, webinars, community presentations, and our Let's Talk series, Caregivers Outreach provides practical information that helps caregivers better understand caregiving, available resources, wellness, planning, and navigating systems of care.
Caregiver Support & Wellness
Caregivers need opportunities to care for themselves, connect with others, and take time away from the demands of caregiving. Our wellness and respite programming includes educational and wellness activities, Family Game Nights, recreational experiences, caregiver appreciation programs, and other opportunities designed to promote connection and well-being.
Private Geriatric Case Management

Personalized Support for More Complex Caregiving Needs
When a family is navigating complex decisions involving an aging loved one, finding a resource may only be the beginning.
Caregivers Outreach offers Private Geriatric Case Management for families who need more individualized professional assistance. Services may include comprehensive needs assessments, individualized care planning, healthcare and long-term care navigation, family consultation, coordination with providers, care transition support, community resource coordination, and ongoing case management.
Our goal is to help families understand their options, develop an appropriate plan of care, and navigate difficult decisions with greater confidence.
Private Geriatric Case Management is a fee-based professional service and is separate from Caregivers Outreach's general resource navigation and referral services.
Community Programs
Connecting Caregivers With Their Community
Strong caregiver support extends beyond individual services. Caregivers Outreach creates opportunities for families to connect directly with healthcare providers, government agencies, community organizations, professionals, and other resources.
Health & Resource Fairs
Caregivers Outreach coordinates three major Health & Resource Fairs each year, bringing community resources directly to caregivers and families in accessible, welcoming environments.
Our fairs connect attendees with healthcare providers, community-based organizations, government programs, caregiver resources, wellness information, screenings, educational opportunities, and other services.
Services for Employers & Organizations
Helping Organizations Better Support Caregivers
Caregiving doesn't stop when someone arrives at work.
Employees may be balancing professional responsibilities while caring for aging parents, spouses, children with disabilities, or other loved ones. Organizations that understand the caregiver experience can better support employees, strengthen workplace culture, and connect their workforce with valuable resources.
Caregivers Outreach provides customized services for employers, healthcare organizations, government agencies, community organizations, and other institutions.
Corporate & HR Caregiver Training
Customized training helps Human Resources professionals, managers, supervisors, and employees better understand the realities of family caregiving and identify practical ways to support working caregivers.
Training can be customized around caregiver awareness, workplace communication, caregiver resources, manager education, caregiver wellness, and building a caregiver-supportive workplace.
